Instructions for Paperwork:

• Please complete both pages of the “New Client Information” form.  (If coming for Couples or Family Counseling, please fill out a form for each individual.)
• The “Counselor/Client Agreement” is for you to read and sign.  Please discuss any questions or concerns you have with your counselor.
• The “HIPAA Notice” is to inform you about HIPAA regulations to protect your privacy.  This form is for you to keep and does not need to be returned to your counselor.
• Please bring the completed paperwork with you to your first appointment.
